PRESCRIPTIONS
Please follow all instructions pertaining to your visit at North Texas Dental Surgery.

| ☐ | Amoxicillin/Clindamycin 500mg 250mg 100mg | 3x/day 3x/day 3x/day | This medication shouldn’t be taken if allergic to penicillin or related medications. It could also interfere with female birth control. Take all prescribed tabs as directed. Take 1 tab 3 times a day till gone. |
| ☐ | Ibuprofen 800mg | 4 tabs/day MAX | This medication shouldn’t be taken if you have kidney/liver problems. Pain medication shouldn’t be mixed with alcohol. Take 1 tab every 6-8 hours as needed for pain. Do not exceed the MAX per day. |
| ☐ | Norco 10mg 7.5mg 5mg | 4 tabs/day MAX 4 tabs/day MAX 5 tabs/day MAX | This pain medication is very strong so food should be taken with it. Do not operate heavy machinery when using this medication. This prescription causes drowsiness. Take as directed only. Do not mix with other pain medications or alcohol. Can cause liver, kidney, and even heart problems if not taken properly. On rare occasions, can even cause death if not carefully taken. Take 1 tab every 6-8 hours as needed for pain. DO NOT EXCEED THE MAX PER DAY. |
| ☐ | Zofran ODT 4mg | 3 tabs/day MAX | This medication shouldn’t be taken with alcohol. Take 1 tab every 6-8 hours as needed for nausea. May cause drowsiness so be careful with driving. |
| ☐ | Tylenol 3
| 4 tabs/day MAX | This pain medication is very strong so food should be taken with it. Do not operate heavy machinery when using this medication. This prescription causes drowsiness. Take as directed only. Do not mix with other pain medications or alcohol. Can cause liver, kidney, and even heart problems if not taken properly. On rare occasions, can even cause death if not carefully taken. Take 1 tab every 6-8 hours as needed for pain. DO NOT EXCEED THE MAX PER DAY. |
